A. WHAT THE EMPLOYEE SHOULD DO IF INJURED ON THE JOB DURING REGULAR BUSINESS HOURS

  1. NOTIFY YOUR SUPERVISOR IMMEDIATELY, no matter how minor the injury may seem. In reporting your injury or disease, your supervisor must complete an On-The-Job Injury Report found on the Office of Human Resources website.
     
  2. If necessary, obtain medical attention. You may choose a doctor, hospital or pharmacy from our Preferred Providers List under the Injury Management Organization Network (IMO) at http://injurymanagement.com/services-2/imo-med-select-network/find-a-provider/.  Once on the site, please look up providers by your nearest zip code within the Cameron, Hidalgo and Star counties. If you live in Willacy county, you do not need to use the IMO Network. The Office of Human Resources will provide you with a Provider Notification of an On-the-Job Injury referral form to give to your medical provider.
     
  3. If you miss work due to an on the job injury (the initial date of injury not included), you must contact the Office of Human Resources to complete additional forms.
     
  4. When your doctor releases you to return to work, you must return to work at the beginning of your next regularly scheduled work day.  The Office of Human Resources must receive a release to return to work from your medical provider.

B. WHAT THE EMPLOYEE SHOULD DO IF INJURED ON THE JOB AFTER REGULAR BUSINESS HOURS

  1. If necessary, obtain medical treatment as may be deemed “reasonable and necessary” at a hospital emergency room or IMO Preferred Provider.
     
  2. NOTIFY YOUR SUPERVISOR as soon as possible. Failure to report the injury within 30 days from the date of injury may result in the denial of a claim. In reporting your injury or disease, your supervisor must complete an On-The-Job Injury Report found on the Office of Human Resources website. Follow instructions #3 – 5 listed in Section A above.
