Designated to help members achieve their individual health goals, Blue Care Connection is an umbrella of condition management and wellness programs that provide BCBSTX members access to outreach, educational resources and health advocacy services through the Personal Health Manager on Blue Access for Members at www.bcbstx.com/ut.
The health consequences of being over-weight/obese or smoking are associated with many chronic medical conditions. By using these programs, members who are ready to lose weight or quit smoking can receive support and utilize tracking tools to measure their progress.
The tobacco cessation program also provides personal coaching, online tools and discounts to wellness-related products and services.
The weight management program offers guidance and support through behavioral and motivational coaching, personalized goal setting with an action plan, online tools and discounts to wellness-related products and services. To get started, members can just click the Weight Loss or Stop Smoking button on the Personal Health Manager home page. For members meeting specific criteria, additional support is available through telephone outreach.
Wellness Coaches-licensed clinical social workers and master level therapists-will provide personal assistance with goal –setting and periodic progress checkups for members who have the greatest need and desire to make a change to their health. Wellness Coaches will call participating members periodically to provide personal assistance with: goal setting, progress checkups, resource education, and motivation and support.
