Location of Library Forms

As you may have noticed, our library forms have moved out of our homepage and into their respective departments. Please see the following for the permanent location of each library form:

• The Class Instruction Form is now located within our Instruction & Tutorials webpage.
• The Interlibrary Loan Request Form is located within our Interlibrary Loan Department webpage.
• The Purchase Request Form is located within our Acquisitions Department webpage.
• The Reserves Forms are located within our Reserves Department webpage. There are three types of course reserves that instructors can request:
  1. Article Reserves
  2. Library Book Reserves
  3. Personal Material Reserves
