Budget Amendment Form

You will need to download and fill out the Amendment Form by clicking on the link at the bottom of this page. Microsoft Excel 97 or higher is required.

Steps to follow:

1. You will need to enter the Item Number, Department Name, Item Description, beginning balance, debit, credit. The form has formulas and will automatically give you the ending balance.
2. Enter the reasons for changes. Please be specific.
3. After entering all the above information from the computer, print out the form, sign it, and submit it to Tandy Suite 100, Room 109.
