WHAT IS Evidence of Insurability?
Evidence of Insurability (EOI) is a record of a person’s past and current health events. EOI is used by insurance companies to determine whether a person meets the definition of good health.
An EOI application is required when you want to:
- Add a spouse or dependent(s) to your medical coverage, unless proof of other active group medical coverage can be provided during Annual Enrollment.
- Reinstate terminated or waived medical coverage, unless proof of other active group medical coverage can be provided during Annual Enrollment.
- Increase your life insurance coverage.
- Increase your spouse’s life insurance coverage.
- Add Long Term Care coverage for yourself, your spouse and all other eligible family members.
If EOI is required:
- You will be prompted to complete an online form when making your benefits selections on the My UT Benefits website.
- If you choose to complete a paper application, you MUST mail your EOI form to the address listed on the application by August 15. Applications postmarked after August 15 will not be accepted.
The EOI requirement for the UT SELECT medical plan will be WAIVED if you can show proof of other active group medical coverage for your spouse or dependent(s).
Acceptable forms of proof include:
- A letter from an employer or plan administrator
- A HIPAA Certificate (Certificate of Group Health Insurance)
- A copy of a check stub that shows a payroll deduction for medical coverage
