FMLA Process
An employee should apply for Family Medical Leave if:
- Off work for 3 or more calendar days due to an illness or injury.
- Or anticipates an absence of 3 or more calendar days due to their illness, injury or pregnancy or that of a qualified family member.
What the Employee/Supervisor should do:
- The employee or his/her supervisor must complete the online application for Family and Medical Leave on the Human Resources website (http://www.utb.edu/ba/hr/benefits/fmla), if practical, at least 30 days prior to the start of the leave or soon as possible. - Medical Certification from a health care provider to support the requested leave is also required if the leave is for an illness/injury of the employee or that of a qualified family member. Completed form can be faxed to (956) 882-6599.
- The department supervisor must send a weekly Staff Absence Report to Human Resources while the employee is on leave. Completed forms can be faxed to (956) 882-6599.
- The department supervisor must notify Human Resources when an employee returns to work from leave. A return to work release from the employee’s health care provider may be required.
FMLA Approval:
- FMLA leave is approved by Human Resources in coordination with the employee’s supervisor for both staff and faculty.
