EH&S Work Orders
University employees requesting the removal of hazardous waste or materials must submit an Environmental Health & Safety work order. Examples of such waste include, but are not limited to, broken glass, toner cartridges, lead acid batteries, UV lamps, hard drives (from Information Technology Services), unused or orphaned chemicals, and chemical and biohazardous waste.
Work orders track compliance of health and safety procedures for handling such material.
The work order includes a field for a work order number. EH&S staff will add that information, after the requestor sends the form. Work order forms can be:
