Note: One of these statements (either Mandatory or Voluntary) needs to be handed to the individual anytime you are asking for their social security number. This statement needs to be read to the individual if the request for their social security number is over the phone.

On all printed forms, the appropriate disclosure can be appended to it. On all re-orders, the statement will need to be printed on the form. Electronic forms must link to the appropriate disclosure statement or the appropriate disclosure statement must be prominently displayed on the website.

 
Directions:
Use below MANDATORY disclosure statement if you are asking for the social security number under a state or federal statute or regulation.
Note: You must state the intended use of their SSN and also whether it is a Federal or a State law/statute/regulation that authorizes the University to require their social security number.

General mandatory disclosure.

Disclosure of your Social Security Number (“SSN”) is required of you in order for The University of Texas at Brownsville and Texas Southmost College to __ [ state intended use of SSN here ] __, as mandated by __ [ Federal ] [ State ] __ law. Further disclosure of your SSN is governed by the Public Information Act (Chapter 552 of the Texas Government Code) and other applicable law.

                  
Directions:
Use the below VOLUNTARY disclosure statement if you are requiring the social security number of the individual but it is not authorized under a particular state or federal statute or regulation.
Note: You must state what you intend to use the SSN for and also what could happen if the individual chooses not to disclose his/her social security number. (Ex: delays in processing their request or application) 

General voluntary disclosure.

Disclosure of your social security number (SSN) is requested from you in order for The University of Texas at Brownsville and Texas Southmost College to __ [ state intended use of SSN here ] __. No statute or other authority requires that you disclose your SSN for that purpose. Failure to provide your SSN, however, may result in __ [ state what may happen if the individual fails to provide SSN here ] __. Further disclosure of your SSN is governed by the Public Information Act (Chapter 552 of the Texas Government Code) and other applicable law.
